The system includes a data capture interface receiving electronic response data of the subject across a set of risk-factor parameters, a weight matrix store outputting a discrete categorical weight value for each response category, a scoring processor combining the weight values into a weighted score, a multi-threshold comparator assigning the subject to one risk category from screening, intermediate, and confirmatory categories, and a routing controller forwarding an electronic referral instruction to a remote capture node or a spirometric confirmation node of a healthcare network. Unlike conventional undifferentiated questionnaires that lack structured downstream routing and remain disrupted by intermittent connectivity, the present arrangement delivers stratified referral, connectivity-resilient capture, and regional-language interaction at primary, peripheral, and community healthcare deployments.
