A microcontroller (102) mounted on the flexible substrate (126) is electrically coupled to the plurality of sensors (104) and configured to receive concurrent sensor data, aggregate a weighted sensor score with each sensor assigned a respective physiological weight, quantify a multi-parameter risk factor from the weighted sensor score of the plurality of sensors (104) concurrently exceeding respective threshold values, and generate a skin barrier integrity score from the weighted sensor score modulated by the multi-parameter risk factor.
