Abstract: [065] The present invention relates to a system and method for intelligent healthcare navigation and continuity management. Symptom information received from a user computing device is transformed into a machine-processable representation and analysed to determine at least one condition-related indicator and an urgency parameter. A healthcare-provider category is determined from the processed information, and one or more corresponding healthcare-service options are generated for facilitating appointment creation. Information relating to the symptom-processing session and appointment is associated with a longitudinal user record. An authorised provider computing device may access relevant appointment and historical information and provide prescription, referral or follow-up information for updating the longitudinal user record. An event and reminder module generates subsequent healthcare-related events from the updated information. When the urgency parameter satisfies an emergency criterion, an emergency-response workflow may be activated using authorised location information. The system thereby provides an integrated computational pathway for healthcare navigation and continuity management. Accompanied Drawing [FIGS. 1-2]
