Abstract: 075] The present invention discloses a system and method for computer-assisted detection and longitudinal evaluation of imaging biomarkers in MRI and/or CT scan data associated with neurodegenerative conditions. Volumetric image data are standardized to compensate for spatial, image-value, resolution, and acquisition-domain variations. Anatomical regions are identified, and a multi- scale deep feature extraction network extracts local, regional, and global imaging features. A biomarker correlation engine processes relationships between anatomically related regions to generate a topology-aware quantitative biomarker representation. Temporally separated scans may be anatomy-constrained registered and compared to generate a longitudinal biomarker-change vector while reducing scanner-dependent variation. An uncertainty estimation engine determines confidence values using image-quality and processing-reliability information. A structured output comprising quantitative biomarker measurements, anatomical location information, longitudinal changes, and associated confidence values is generated for computer-assisted professional assessment of structural changes relevant to neurodegenerative conditions.
