Abstract: Insurance policy administration is typically handled either by heavyweight commercial-off-the-shelf (COTS) suites that are expensive to configure and validate, or by ad hoc spreadsheets and manual processes that provide no enforced lifecycle control and no audit trail linking requirements to tested behavior. Neither approach makes it easy to demonstrate, for a regulator or an internal quality function, that every documented requirement was actually implemented and tested. This system addresses that gap with a lightweight web application that enforces a strict policy and claims lifecycle (DRAFT -› ACTIVE, and claim PENDING APPROVED/REJECTED) directly in the data model and route logic, so an invalid transition (e.g., filing a claim against a non-active policy, or editing a policy no longer in DRAFT) is rejected by the system itself rather than relying on user discipline. Alongside the application, the project includes a documented User Requirements Specification (URS), a formal Installation/Operational Qualification and User Acceptance Test protocol (1Q/0Q/UAT), and a script that automatically generates a requirement sto- test-case traceability matrix as a spreadsheet — giving the project a self-documenting path from stated requirement to verified test result, in the style expected of validated software in regulated environments.
