Abstract: ABSTRACT Conventional ligature wire twisting in fixed orthodontic therapy is performed manually using a Mathieu plier or a needle holder. Although effective, this technique is repetitive, time consuming, and prone to operator fatigue, inconsistent twist tension, sharp pigtail formation, and accidental wire breakage. Inconsistent ligature twists also contribute to soft tissue irritation and premature loosening of the ligature wire, requiring early replacement. To overcome these limitations, the present invention provides a smart, motorised ligature twisting device that retains the familiar ergonomics of a conventional Mathieu plier while integrating a compact micromotor and gear train within the central body of the instrument between the two spring handles. Activation of the micromotor through a thumb operated control switch transmits torque through a gear assembly to a rotating twisting head positioned behind the gripping beaks. The rotating head delivers controlled, uniform 360- degree rotation of the wire ends held by the serrated beaks, producing reproducible ligature twists with predetermined tension and minimal operator effort. A central hook positioned at the rotating head facilitates engagement and clean release of the twisted ligature wire. The device retains the standard Mathieu handle and ratchet locking mechanism, ensuring tactile feedback, autoclavability of the working tip, and seamless integration into existing orthodontic workflow. The invention represents a meaningful advancement over the manual ligature twisting technique by improving treatment efficiency, ergonomics, and the consistency of orthodontic ligation.
