Abstract: Abstract Medication non-adherence among elderly patients and individuals with chronic illnesses remains a major healthcare challenge, often leading to missed doses, incorrect intake timing, duplicate consumption, and severe medical complications. Conventional pill reminder systems provide only time-based alerts and fail to verify whether the correct medicine has been consumed. This invention proposes an AI-Powered Smart Medicine Box with Dose Verification and Elder Compliance Prediction, designed to ensure safe, accurate, and predictive medication management. The proposed system integrates IoT-enabled compartment sensors, camera-based pill verification, voice-assisted reminders, and AI-driven adherence analytics to monitor real-time medicine usage. Each compartment is equipped with smart sensors to detect opening events, pill removal count, and scheduled dose timing. A built-in camera module verifies the type, color, and shape of the pill to ensure the correct medication is taken. In addition, voice prompts in regional and multilingual formats assist elderly users and request confirmation after consumption. The AI engine continuously analyzes historical medicine intake patterns, missed-dose frequency, delay intervals, and user behavior to generate an elder compliance prediction score, identifying the probability of future non-adherence or accidental overdose. When abnormal behavior is detected, such as skipped medication, duplicate dose attempts, or irregular timing, the system instantly sends alerts to family members, caregivers, or healthcare providers through a connected mobile application. The proposed framework provides a privacy-aware, low-cost, and intelligent healthcare assistance solution for elderly care, chronic disease management, and home-based patient monitoring. By combining real-time verification with predictive AI analytics, the system improves patient safety, treatment effectiveness, and independent living support.
