Abstract: ABSTRACT The present invention relates to a convertible pediatric dental appliance that combines anterior esthetic tooth replacement and posterior space maintenance in a single fixed appliance for the management of premature loss of multiple primary teeth. The appliance comprises an anterior prosthetic component for replacement of missing primary incisors and bilateral posterior space-maintaining extensions adapted to preserve arch length following the premature loss of primary molars. The posterior extensions are configured to engage the primary canines and posterior abutment teeth, thereby maintaining space for the eruption of permanent successors. A unique feature of the invention is its convertible design, which permits selective removal or modification of the anterior esthetic component upon eruption of the permanent incisors without disturbing the posterior space-maintaining framework. The remaining appliance continues to function as a fixed space maintainer until eruption of the permanent premolars eliminating the need for complete appliance replacement. The invention provides improved esthetics, functionality, patient comfort, treatment efficiency and long-term space management during the mixed dentition period.
