The device comprises an upper tray for receiving maxillary teeth, a lower tray for receiving mandibular teeth, and a central rectangular spacer section positioned between the upper and lower trays to maintain a fixed inter-arch separation and a radiation beam-clear region. A tongue shield is integrated with the central rectangular spacer section and extends toward an anterior tongue-contact region to hold the tongue away from a radiotherapy field. A saliva drainage channel is formed along a base region of the spacer body. A click-lock stabilization assembly is disposed at lateral side portions of the spacer body to retain repeatable jaw positioning during head and neck radiotherapy.
