Abstract: The present invention discloses a plantar perfusion monitoring device (100) for early detection of diabetic foot ulcer. The device (100) comprises a foot sole (201) embedded with a plurality of sensors (205) comprising sensor bundles (205A, 205B, 205C, 205D, 205E) positioned at specific plantar zones, each sensor bundle in optical alignment with a corresponding optical window of a plurality of optical windows (203) to detect pulsatile blood-volume data at the plantar skin surface. A processing unit (211) determines a perfusion index, generates a patient-specific baseline perfusion profile, and generates an alert upon the determined perfusion index falling below or exceeding a threshold perfusion index. Determined perfusion index and deviation-based alerts are wirelessly transmitted via a communication interface (213) to a remote monitoring interface (217), facilitating early clinical intervention prior to visible tissue breakdown.
