Abstract: An orthopedic instrument system for controlling osteotomy distraction, customizing bone grafts, and preventing lateral hinge fractures (LHF)—specifically avoiding Takeuchi Type III intra-articular lateral tibial plateau fractures—during opening-wedge high tibial osteotomy (OW-HTO) is disclosed. The system comprises a main measuring plate ("MAIN") featuring a linear scale with progressive metric increments and sliding guide pins configured to track gap expansion in real time. The system further includes a plurality of modular, sequentially indexed sizing plates (1–5) provided with variable-length longitudinal tracking slots that decrease progressively across the series. These modular plates are configured for bi-directional, alternating layered insertion across an osteotomy site to distribute transverse friction forces evenly, eliminate axial twisting, and prevent single-sided canting during distraction, thereby protecting the intact lateral cortex. Additionally, the tracking slots serve as rigid ex-vivo cutting templates corresponding directly to the measured gap. Trimming a bone graft within the boundary of a selected sizing plate ensures a sub- millimeter geometric match with the distracted void, eliminating aggressive graft impaction forces, preventing lateral hinge disruption, and protecting against early conversion to knee arthroplasty.
