Abstract: The present invention relates to a mobile Internet of Things (IoT)-enabled diabetic foot ulcer detection and severity classification system configured to provide automated diagnosis, severity assessment, and remote healthcare monitoring using deep learning and explainable artificial intelligence. The system comprises an image acquisition module, an image preprocessing module, a YOLOv8-based ulcer detection module, an EfficientNet-B0 severity classification module, a Grad-CAM explainability module, an IoT communication module, a cloud database, and a web-based user interface. Initially, a foot image is captured using a mobile device and preprocessed to improve image quality. The processed image is analyzed by the YOLOv8 model to accurately localize the ulcer region, which is subsequently classified into predefined severity levels using the EfficientNet-B0 deep learning model based on the Wagner grading system. The Grad-CAM module generates visual heatmaps highlighting clinically significant regions influencing the classification decision, thereby improving model transparency and clinical interpretability. The diagnostic results are securely transmitted through IoT communication to cloud-based healthcare platforms, enabling remote patient monitoring, telemedicine consultation, and long-term disease management. The invention provides a reliable, scalable, and clinically applicable diabetic foot ulcer diagnosis system that improves diagnostic accuracy, supports early medical intervention, reduces the risk of severe diabetic complications, and enhances healthcare accessibility through intelligent medical image analysis and mobile IoT technologies.
