Abstract: A mitochondrial gene therapy specifically designed for the management of diabetes by targeting the MT-ND1 gene within pancreatic beta cells. This therapy utilizes a viral vector, selected from lentiviruses, adenoviruses, or adeno-associated viruses, engineered for precise mitochondrial targeting to deliver therapeutic genetic material directly to the mitochondria. The genetic material comprises a corrected or enhanced version of the MT-ND1 gene, crucial for the function of Complex I in the mitochondrial electron transport chain. The integration of this genetic material into the mitochondrial DNA is achieved through homologous recombination, ensuring stable expression and correcting mutations or enhancing gene expression. This process restores Complex I function, thereby improving ATP production, which in turn enhances insulin secretion and glucose metabolism in diabetic patients. The therapy is adaptable to individual genetic mutations in mitochondrial DNA, and administration routes include intravenous, intraperitoneal, or direct pancreatic injection, with patient monitoring for improved beta cell function.
