Abstract: A system (100) for determining blood oxygen saturation in an intravascular region (130) distal to an occlusion in a vascular system, the system comprising a processing device and an interventional device, the processing device comprising an optical interface for transmitting broadband light to the interventional device and for receiving (S110) optical data from the interventional device, the optical interface configured to optically couple to a proximal optical interface of the interventional device, the interventional device comprising an elongated body with a diameter less than 0,018 inches, the elongated body comprising an optical fiber (310) for transmitting broadband light to a distal optical interface and to transmit optical data from the distal optical interface to the proximal optical interface the distal optical interface configured to directly optically couple the optical fiber (310) with the intravascular region (130) such that the optical data comprises radiation covering an optical spectrum over one or more wavelength intervals reflected by tissue being analyzed in the intravascular region (130). The processing device further comprises at least one optical detector for measuring the optical data and generating measurement data (120) and one or more processors (110) configured to receive (S110) measurement data (120) and analyze (S120) the measurement data (120) to determine a measurement of blood oxygen saturation (140); and output (S130) the measurement of blood oxygen saturation (140). Preferably an optical model is used to fit the optical spectrum to determine the blood oxygen saturation or an AI model trained on in vitro blood oxygen saturation measurement is used.
