Abstract: [042] The present invention discloses an intelligent IoT-cloud system for remote patient monitoring, telemedicine and AI-assisted personalized health risk assessment. The system acquires multimodal physiological, behavioural and contextual data through distributed patient-side sensors and associates observations with sensor- confidence metadata. An edge gateway performs validation, adaptive sampling, local buffering and risk-dependent selective transmission to a secure cloud platform maintaining a dynamically updated patient-specific digital health twin. A temporal AI risk engine evaluates deviations from individualized baselines and produces a structured risk state including deterioration risk, time horizon, inference confidence and contributing trends. A counterfactual consistency module evaluates whether elevated risk persists after attenuation or exclusion of low-confidence observations, thereby enabling repeat-measurement requests before unnecessary clinical escalation. Qualified alerts are supplied to a telemedicine interface with a risk timeline and supporting context. Clinician disposition and validated outcomes are used to recalibrate patient-specific risk parameters, sensor-reliability weighting or sensing policies, thereby forming a closed-loop monitoring and intervention architecture.
