Abstract: The present invention discloses an inhalable pharmaceutical composition comprising tepotinib-loaded exosome-mimetic nanovesicles for the targeted treatment of MET exon 14 skipping mutation-positive non-small cell lung cancer. The exosome-mimetic nanovesicles are produced by serial extrusion of mammalian cells through polycarbonate membranes of 10, 5, and 1 micrometre pore sizes and loaded with tepotinib via sonication-assisted incubation. The formulation, optimized using a Box- Behnken design, demonstrates encapsulation efficiency of 91.56 percent with vesicle size of 148.2 nanometres. The nebulized formulation enhances bioavailability by 6.94-fold, increases lung targeting by 8.99-fold, and extends mean dissolution time by 3.21-fold compared to oral tepotinib. It reduces the IC50 against A549 cells by 4.7-fold while maintaining safety in normal lung cells. In vivo studies confirm superior anti-tumor, anti-inflammatory, and anti-oxidative efficacy with a safe profile at 100 milligrams per kilogram.
