Abstract: A stable, alcohol-free, and chlorhexidine-free herbal mouthwash composition for oral healthcare applications is provided. The composition comprises a methanolic extract of Balanophora fungosa (0.15% to 0.2% w/v) as the primary botanical active component. This extract is integrated into a cooperative carrier matrix consisting of glycerine (10% to 16.67% v/v) as a humectant and stabilizing co-solvent, xylitol (0.67% to 1.0% v/v) as a sweetening and antibiofilm-enhancing component, Fast Green FCF (0.033% to 0.067% v/v) as a colouring component, and an antiseptic and flavouring component selected from peppermint, eucalyptol, and combinations thereof (0.67% to 1.0% v/v) in distilled water as an aqueous vehicle. This matrix solubilizes and stabilizes lipophilic bioactives without phase separation. The resulting homogeneous oral rinse exhibits synergistic broad-spectrum antimicrobial activity with a minimum inhibitory concentration of 0.9 p.g/mL against oral pathogens, multi-mechanistic antioxidant properties, and preformed biofilm- disrupting efficacy against Streptococcus mutans
