Abstract: Title: Bio-Adaptive Pneumato-Dynamic Smart Vaginal Dilator with Physiological Feedback-Controlled Pressure Regulation and Active Microfluidic Cleansing The current technology pertains to a bio-adaptive pneumato-dynamic smart vaginal dilator (117) for postoperative maintenance of a surgically created or reconstructed vaginal canal. The dilator comprises a modular architecture including a reusable smart core and a replaceable biofluidic sleeve (117), a body-temperature-activated shape-memory polymer core (118), an auxetic radially expandable pneumatic sleeve (108) having independently controllable pneumatic chambers (109), electromyographic (EMG) sensors (104), photoplethysmographic (PPG) sensors (106), a dual-lumen microfluidic irrigation and vacuum evacuation system (101) and a processing unit (119). The processing unit dynamically regulates localized pneumatic pressure in response to pelvic-floor muscle activity and reduced mucosal tissue oxygenation or perfusion. The microfluidic system (101) delivers a therapeutic, antimicrobial or clinician-selected cleansing fluid while simultaneously evacuating accumulated biological fluids and debris (113). Physiological and treatment-compliance data are securely transmitted to an external monitoring application. The innovation is configured to improve patient comfort, promote tissue healing, reduce pressure-related complications, minimize infection risk and support long-term postoperative rehabilitation.
