Abstract: ABSTRACT ARTIFICIAL INTELLIGENCE-BASED MEDICATION THERAPY MONITORING SYSTEM FOR REAL-TIME DETECTION AND PRIORITIZATION OF DRUG-RELATED PROBLEMS The present invention relates to an artificial intelligence-based medication therapy monitoring system (100) for detection and prioritization of drug-related problems. The system comprises a patient-data acquisition module (102), medication-profile generation module (112), data integration and preprocessing module (110), AI-based drug-related problem detection engine (116), medication-risk scoring module (130), severity determination module (132), intervention prioritization module (134), explainability module (136), pharmacist interface (138) and intervention-record generation module (140). Prescription information, medication history and patient-specific parameters are integrated into a patient-specific medication profile (114) and computationally analyzed to detect a plurality of drug-related problems. Patient-specific medication-risk and severity information are determined for the detected problems and used to generate a prioritized drug-related problem output (152). The explainability module generates information associated with detection and/or prioritization of respective problems for pharmacist review. The intervention-record generation module generates an electronic pharmacist intervention record (154) corresponding to a reviewed drug- related problem. The system enables integrated, prioritized and longitudinal monitoring of patient-specific medication therapy.
