Abstract: The present invention relates to an electronic health record (EHR) management system for secure, decentralized storage, access, and sharing of patient medical data. The system addresses limitations of traditional centralized EHRs by enabling patients to control access to their records, ensuring privacy, security, and transparency. Patient data is encrypted using hybrid encryption, stored off-chain on the Interplanetary File System (IPFS), and managed via smart contracts deployed on an EVM- compatible blockchain. Upgradable smart contracts allow software improvements and feature additions without data loss, while gas-less meta-transactions simplify user interaction. The system provides interfaces for patients and healthcare providers to upload, access, and manage medical records according to permissions set by patients. Immutable audit trails, distributed storage, and cryptographic verification ensure data integrity, fault tolerance, and tamper-proof operations. The invention facilitates telemedicine, prescription management, and health tracking, offering a secure, patient-centric, and scalable framework for electronic health record management.
