Abstract: ABSTRACT OF THE INVENTION ABSTRACT An AI-enhanced virtual health coach for dynamic insulin dosage titration includes a conversational AI module (LLM with optional RAG) that engages a diabetic patient in natural dialogue to obtain glucose measurements and contextual information. A deep reinforcement learning (DRL) personalisation module processes the patient's real-time CGM data, wearable sensor data, and self-reported data to determine an optimal insulin dosage. A digital twin simulation engine may test the proposed dosage virtually before real-world implementation. Federated learning ensures privacy-preserving collaborative model improvement. A blockchain provides an immutable audit trail. An electronic signal is generated to cause an insulin delivery device (pump or smart pen) to administer the recommended dosage. Optionally, a VR headset is triggered to reduce injection-related pain and anxiety. The system improves glycemic control, adherence, and safety while meeting modern regulatory and privacy requirements.
