Abstract: The present invention concerns an Artificial Intelligence (Al) supported system for gathering, structuring, validating and creating patient case histories prior to or during consult. Patient information is collected via text, voice and multilingual interactions and the following questions are dynamically generated based on the information gaps, uncertainty and clinical relevance of the previously collected information. A clinical completeness map classifies collected data by its completeness level (e.g., complete/incomplete/uncertain contradicting) for chief complaints, history of present illness, past medical history, surgical history, medication history, allergy history, family history, personal history and review of systems. A multilingual normalization engine can be used to accurately map regional-language answers and colloquial phrases into the standardized clinical concepts used in the clinical domain, while preserving the patient's answer to ensure it is the actual answer being compared. The system also includes temporal symptom mapping, contradiction detection, and confidence assessment and intelligent re-questioning. A structured case summary is only created when rules for validation have been applied and it offers a doctor controlled interface for reception, editing, rejection and clarification of information. The system can send non-diagnostic priority alerts according to the predefined or configured clinical information patterns. The invention thus provides a reduction in manual documentation of repetitive work, enhanced consistency of collecting patient histories, enhanced communication in multiple languages and a structured, reviewable and editable clinical history without being able to diagnose the patient or treat the patient.
