Abstract: Title: Acute Ischemic Stroke Transfer Decision Support System and Method Thereof The disclosure provides a clinical decision-support system for spoke-to-hub transfer triage of acute ischemic stroke using non-contrast computed tomography (NCCT) (107), contemporaneous blood glucose (108) and symptom-onset timing (109). The system anatomically maps cerebral territories (102, 111), associates regional Hounsfield-unit measurements with affected ipsilateral and corresponding contralateral reference tissue and calculates raw Net Water Uptake (NWU) (112). A dynamic Glucose Correction Factor (GCF), determined from the patient's blood- glucose level (108) relative to a reference concentration, is applied to generate glucose-corrected NWU (113). Corrected regional values are weighted according to neurological eloquence (103, 114), aggregated across relevant territories and integrated with a temporal parameter (116) and optionally an NCCT-derived anatomical or infarct-related parameter (115) to generate a bounded 0–100 Glucose-corrected Net-Water-Uptake Transfer Score (GTS). The GTS is used to generate a patient-level categorical transfer decision-support output (117), enabling quantitative stroke triage in resource-limited healthcare networks without requiring perfusion CT, MRI or immediate on-site specialist interpretation.
