Abstract: The present invention discloses a hardware-tied pharmacy adverse drug reaction signal detection system (10) for converting routine dispensing and counselling records into pharmacist-reviewable adverse event reports. A pharmacy dispensing terminal (1000) feeds prescription history, refill timing, medication changes, complaint text and OTC co-purchase data to a structured database (1100) through an event stream buffer (1300). An ADR detection engine (100), NLP engine (200), drug-event dictionary (300) and temporal causality module (400) map complaints to MedDRA-style terms, compare onset timing with drug start and dose-change events, and generate suspected drug-event pairs with causality scores. A dechallenge/rechallenge capture module (500), duplicate-signal suppression module (600), temporal window calibration register (1600) and OTC interaction context filter (1900) refine signal quality. A pharmacist dashboard (700) displays evidence, approvals and feedback. FAERS/MedWatch field integration (800), report generation hardware workflow (1200), local encryption controller (1500) and report hash/versioning unit (1800) generate secure draft reports before controlled report finalization.
