Abstract: ABSTRACT A SYSTEM AND METHOD FOR HOME-BASED MONITORING AND ADAPTIVE ESCALATION OF POST-ALLOGENEIC HSCT PATIENTS Clinical experience with a structured home-based monitoring and care protocol after allogeneic HSCT demonstrated the feasibility and safety of supervised post- discharge care, supported by caregiver training, symptom reporting, nurse home visits, line care, laboratory sampling, consultant review, complication detection and predefined hospital escalation. The present disclosure relates to an AI-driven digital remote monitoring, predictive risk stratification and adaptive escalation system for post-allogeneic HSCT patients. The system integrates patient engagement, wearable-device, laboratory, nurse workflow and central-line image acquisition modules with digital twin generation, temporal risk correlation, healthcare knowledge graph, sensor fusion analytics, predictive complication forecasting, federated learning, explainable clinical intelligence, dynamic risk-threshold optimization and escalation orchestration engines. Multi-source data, including physiological measurements, laboratory biomarkers, medication adherence, nurse observations, symptoms and imaging inputs, are processed to generate patient-specific risk assessments. The invention enables early detection of infection, GVHD, electrolyte imbalance, central-line complications and readmission risk, supporting timely interventions and improved post-transplant care.
