Abstract: The present invention relates to the field of biomedical signal processing and wearable physiological monitoring, and addresses the technical problem of non-invasively and continuously quantifying cardiac ischemic stress without dependence on clinical infrastructure or cloud connectivity. A system and method for determining an Ischemic Stress Index (ISI) from photoplethysmography signals is disclosed. A dual-wavelength optical sensor acquires a PPG signal at a finger, wrist, or earlobe, which is locally filtered and processed by an embedded microcontroller to extract Heart Rate, Heart Rate Variability, Oxygen Saturation, and Heart Rate Recovery parameters. Each parameter is normalized using predefined baseline and critical thresholds. The system automatically detects a Rest Mode or Recovery Mode and applies corresponding state-dependent weighting factors to compute a composite Ischemic Stress Index on a continuous 0–100 scale, categorized into Low, Moderate, High, and Critical stress levels, displayed locally through an offline, edge-based architecture.
