Abstract: ABSTRACT A SYSTEM AND METHOD FOR CONFIDENCE-GATED MULTIMODAL CLINICAL DECISION SUPPORT WITH EVIDENCE REACQUISITION The present invention generally provides a system and process for generating a confidence-gated clinical decision-support output for a specific patient. The system includes receiving of time-varying physiological signals, medical-image information, and electronic health record information with timestamps. There is an event- anchoring engine to correlate multimodal evidence with an index clinical event and there is a modality integrity engine that computes reliability values using signal artifacts, image quality, temporal validity, and evidence provenance. A processor creates a patient-specific causal evidence graph and generates a candidate decision- support output. There is a counterfactual validation engine that recomputes the output by selectively excluding or perturbing evidence in individual modalities to detect discordance and stability of the output. If the release condition is not met, a closed loop controller sends a targeted evidence reacquisition instruction. A reacquisition of the evidence updates the graph and triggers revalidation. A decision-release gate releases the output when reliability, concordance, and stability conditions and a machine verifiable evidence capsule are met.
