Abstract: Introduction: The “12 Step Eye Test” is a comprehensive procedure designed to ensure a thorough examination of ocular health and visual acuity. This systematic approach covers essential aspects of eye care, from initial hygiene practices to detailed vision assessments and prescription verification. Each step is crucial for diagnosing potential issues and providing accurate corrective measures, ultimately contributing to optimal eye health and patient satisfaction. Methodology: Hygiene: Clean hands and equipment thoroughly. Greetings, introduction, and history check: Greet the patient, provide a short self-introduction, and review the patient’s medical and ocular history. Vision Check: Assess vision at near and far distances. Perform a pinhole acuity check to find out the best achievable visual acuity. Torchlight Test: Examine the eyes using a torchlight. Includes evaluation of ocular adnexa and anterior segment of the eye. Broad H and Cover and Uncover Test: Perform the broad H test to check ocular motility. Perform the cover and uncover test to diagnose ailments of eye alignment. Document and advise as diagnosed. Convergence Test: Test the eyes' ability to converge; repeat 3 times. Objective Refraction: Use a retinoscope or an autorefractometer to measure refractive error. Pupillary Distance Adjustment: Measure the pupillary distance and adjust the pupillary distance in the trial frame. Subjective Refraction: Perform the subjective refraction and document the subjective acceptance. Refine the prescription using a Jackson Cross Cylinder (JCC) and duochrome tests as a verification process. Perform Duochrome to find out the spherical endpoint. Use the cross-cylinder first for the refinement of the cylindrical axis, then the verification of cylindrical power. Perform the Duo Chrome again for the confirmation of the cylindrical adjusted spherical power output. Near Vision: Check near vision acuity, considering the binocularity near reading distance and near vision triad (convergence, pupil constriction, accommodation). New Rx Verification: Verify the new prescription inside the clinic by evaluating the mobility and comfort. Prescription and Consultation: Provide the final prescription and consultation. Further handover to a dispensing optician. Conclusion: By following the “12 Step Eye Test,” eye care professionals can deliver precise and effective vision care.
