Abstract: The present invention a pharmacist-supervised adaptive dose titration system comprises a processor (2400), memory (2500), a communication interface (2600), a data normalization layer (200), a wearable feature extractor (400), a pharmacokinetic and pharmacodynamic digital twin (300), a dose-change simulator (1000), a safety-rule engine (900), and an approval gate (1400). An event synchronization buffer (1500) aligns wearable and laboratory events, rejects duplicate or stale packets, and supplies verified covariates. A model-drift monitor (1700) inhibits simulation when prediction residuals exceed a stored threshold. A dose-step constraint module (1800) limits candidate changes and retains an unchanged-dose option. A physiological exception detector (1900) blocks proposals associated with abnormal wearable trends. A secure release controller (2000) and default-closed communication interlock (2300) prevent patient communication until authenticated pharmacist or physician approval is received. A locked audit trail (1200), hardware security module (2100), provenance registry (1600), and rollback state store (2200) provide tamper detection, traceability, and recoverable model-state control securely.
