Abstract: ABSTRACT A MICRO-CHAMBERED PLANTAR INSERT SYSTEM FOR THE NEUROPATHIC FOOT A micro-chambered plantar insert system for preventing neuropathic and diabetic foot ulcers is disclosed. A pressure-redistribution layer (11) of discrete compressible biomedical-polymer micro-domes (height 1–5 mm, diameter 2–10 mm) in a lattice redistributes axial plantar load laterally between adjacent domes, reducing peak pressure. Capillary channels (12), 100–300 µm wide and 50–200 µm high, co-located in the dome interstices and coupled thereto, allow dome compression to pump perspiration to the periphery by capillary action. A user- replaceable bioactive pad (14) of chitosan and/or poly(vinyl alcohol) loaded with Piceid conditions the plantar micro-environment by antioxidant, anti-inflammatory and anti-biofilm action, complemented by the chitosan matrix. A retention sleeve (10) holds the insert without therapeutic compression. The three functions act simultaneously to establish a controlled plantar micro-environment. Fig. 1
