Abstract: The present invention relates to a digital health consultation facilitation system providing homebased healthcare services through Consultation Facilitation Assistants and registered medical practitioners. Whereas for application in the rural areas, the Consultation Facilitation Assistants shall be referred as Health Tech Mitra. The system integrates patients, assistants, doctors, Medical Delivery Centres, Radiologist Section and the Pathological laboratories using Smart techniques/ Health Cards, workflow applications, doctor consultation interfaces, an AI triage engine, and centralized electronic health record storage. The Certified assistants / Health Tech Mitra conduct home visits with medical diagnostic kits, capture vital parameters, record symptoms, and initiate real time consultations. This may be executed at the Health Kiosk in rural areas, A Smart Health Card with encrypted QR code ensures secure patient identification and access to longitudinal records. The system includes a centralized server architecture with database, API gateway, AI processing, encryption protocols, and role based access control. The AI triage engine analyzes symptoms, vitals, history, and images to generate severity alerts. The platform supports laboratory sample collection, medical imaging integration, geospatial assistant allocation, and structured communication, enabling efficient, standardized, and secure telehealth service delivery for urban communities and improved accessibility.
