Abstract: The present invention relates to a clothing-integrated wearable monitoring system for cognitively impaired individuals comprises a garment (102) having an attachment region (106) configured to receive a tracking module (104) housed within a detachable casing (108). The tracking module (104) includes a location detection unit (110), a communication module (116), a microcontroller unit (118), a tamper detection sensor (120), a motion sensor (142), and a power management module (144). The microcontroller unit (118) processes geographic coordinates obtained from the location detection unit (110) and compares the coordinates with predefined geofence boundary parameters to detect entry into a boundary breach region (122). Upon detection of the boundary breach region (122), an alert escalation system (130) generates notifications through the communication module (116). The communication module (116) supports signal transmission through an RF transmission path (124) and mesh relay through a mesh network node (140), including notification to an emergency response center (136). The power management module (144) provides motion-triggered activation and low-power operation for autonomous monitoring and alert generation.
