Further, the method includes aggregating diagnostic data, IoT wearable streams from an IoT device (110), and historical records into organ-specific repositories, enabling longitudinal tracking of health parameters across normal, borderline, and abnormal thresholds. Further, the method includes determining a trend of health parameters derived from the data aggregated. Further, the method includes generating at least one of a self-care recommendation, a specialist guidance, and an escalation alert based on the trend of health parameters obtained using a machine learning (ML) model."
