Insights are delivered to a patient dashboard (110) and clinician dashboard (112) to enable real-time feedback, remote monitoring, and proactive interventions. The modular system (100) is scalable across post-surgical and chronic care applications, improving adherence, reducing readmissions, and enhancing clinical outcomes while maintaining data security and privacy compliance."
