The system collects medical data from many different places, makes sure that the data is semantically consistent, and then builds a dynamic patient cognitive profile based on the patient's unique traits. The system uses the cognitive profile to figure out a context-aware visualization strategy. It then uses semantic medical abstraction to turn raw clinical parameters into medically meaningful representations that show health status, risk, and progression. The system also makes adaptive visual medical narratives, sets up a traceable interpretation chain that connects source data to generated outputs, and automatically makes medical reports for different stakeholders from a single dataset. The system also makes sure that data access, audit logging, and following the rules are all based on consent. The system and method described here offer a real technical solution that makes medical data easier to understand, more transparent, and easier to use, all while supporting patient engagement, clinical decision-making, and legal reliability."
