Abstract: A self-aware wearable healthcare system is disclosed for establishing a longitudinal and personalized health profile by integrating verified historical clinical information with physiological measurements acquired from a wearable sensing unit. The system generates an individualized physiological baseline and receives authenticated treatment events from an authorized source. A treatment-response traceability engine synchronizes each treatment event, on a shared time reference, with physiological states acquired before and after the event from measurements satisfying a predefined signal-quality criterion, and computes a deviation score normalized against an individualized variability measure. A traceable treatment-response record is generated containing treatment information, source/provenance information, the synchronized event time, physiological states, baseline information and the deviation score. The system may further maintain a secure health-data access trail and identify unexpected treatment-response patterns for authorized professional review. The architecture provides technical synchronization and traceability between historical health information, treatment events and wearable physiological measurements, thereby supporting longitudinal healthcare monitoring and informed clinical review.
