Abstract: Type-2 Diabetes Mellitus is a global health concern with various approaches for regulation. Targeting the glucokinase enzyme, pivotal for glucose sensing and insulin release, is promising. However, glucokinase activators (GKAs) face limitations like hypoglycemia and liver issues. A blend of tetrahydropalmatine and berberine, compounds found in Berberis plants, shows potential as GKAs to address these challenges. Present study highlights and proves its efficacy in regulating glucose levels and improving lipid profiles in diabetic rats over 28 days. The combination enhances glucokinase activity, maintains insulin levels, and mitigates berberine's side effects. Outperforming individual compounds and metformin, a standard anti-diabetic drug, the blend presents a well-tolerated option for type- 2 diabetes mellitus. This novel combination of tetrahydropalmatine and berberine and their synergistic mechanism could fill a gap in managing the disease, offering a promising addition to anti-diabetic medications.
